Understanding what families want from their provider services 

To ensure Nottinghamshire County Council are commissioning Short Break provider services that meets the needs of children, young people and their families we want to hear your views on developing potential provision in Nottinghamshire.

Your views will be used to inform existing and new Short Break providers to help shape and develop services with the views of children and young people and their families in mind.  

We want to carry out this piece of work annually to ensure the changing needs of families are captured and represented in the services we offer. 

The survey should take no longer than 5/10 minutes to complete.
