Testing and Vaccination Uptake Survey

Overview

If you are in receipt of a Direct Payment or Personal Health Budget and employ your own staff, we would be very grateful if you could complete this short survey, which should take less than 15 minutes.  The results will help us to understand more about testing and vaccination uptake and the barriers Personal Assistants (PAs) are facing. This will also support communication with PAs on these issues.

All responses are completely confidential and no information from individual surveys will be shared beyond the Care Sector Testing Operational Group or made public.
