This survey is part of the work being completed across Nottingham and Nottinghamshire to raise awareness of Advance Care Plans for people with dementia.

This survey is for people with lived experience of dementia, including people living with dementia, or those who have experience of being close to or providing care for someone with dementia.

You do not have to complete this. Your time is much appreciated.
