Integrated Children’s Disability Service (ICDS) Education Health Care Plan (EHCP) Parent Satisfaction Survey

Overview

Once an Education Health Care Plan is agreed and issued, the local authority has a statutory duty to ensure the plan is reviewed as a minimum every 12 months. For those under the age of 5 years the plan must be reviewed every 6 months. The Annual Review gives families and professionals an opportunity to consider the child or young person’s support needs and identify where significant amendments to the plan are required.

This Satisfaction Survey forms part of an internal review of the Education Health Care Plan Annual Review processes and documentation for Nottinghamshire only. The survey is to gather your opinion about your family’s experience of the Annual Review process and documentation relating to the education health care plan.

Your views are important as they will help us to understand what parts of the process are working well, and the areas that can be further improved.

Please answer the survey questions as honestly as you can in relation to the child or young person that you care for. There are no right or wrong answers. 

The survey is for parents or carers of a child or young person who has experienced an Education Health Care Plan Annual Review in Nottinghamshire.

Data Protection

Personal data supplied on this form will be used in accordance with Nottinghamshire County Council’s Privacy Policy. The information you provide will be used for statistical analysis, management, planning and the provision of services by the County Council. Nottinghamshire County Council will not share any personal information collected as part of this survey. Anonymised results may be shared with partner agencies to help improve the process in future.
