Overview

Nottinghamshire County Council’s Integrated Children’s Disability Service (ICDS) has commissioned an internal review to look at streamlining the current processes and documentation that support the consultation and review of Education Health Care Plans (EHCP). Its purpose is to improve the EHC Journey for parents/carers and their children or young person, as well as ensuring the timely identification of appropriate educational placements

This Satisfaction Survey is to gather your opinions and experiences relating to the EHC Consultation and Annual Review processes for children and/or young people in your care.

Your views are important as they will help us to understand what we are doing well, your challenges, and the parts of the process that require improvement.  Please answer the questions as honestly as you can in relation to your experience of the Consultation and Annual Review processes. There are no right or wrong answers.

Personal data supplied on this form will be used in accordance with Nottinghamshire County Council’s Privacy Policy. The information you provide will be used for statistical analysis, management, planning and the provision of services by the County Council. Nottinghamshire County Council will not share any personal information collected as part of this survey.
