Covid-19 impact questions for parent and carer survey

Overview

If you have a child or children aged between 1-6, we would like to find out what impact Covid-19 had on you. As part of the recovery from the pandemic we are keen to explore the impact it had on pregnancy, parents and carers and their young children living within Nottinghamshire.

We would be grateful if you were able to answer the below questions, which should take around 15 minutes to complete.

By completing the questions, you will be in with the chance of winning one of three available £20 high street gift vouchers.

Your response to the survey is anonymous and all information is held confidentially and securely. Personal contact information will only be used for the purpose of the prize draw and you do not need to provide this if you would prefer not to enter.

The results will be used to inform our Nottinghamshire covid-impact assessment work and help shape our recovery plans. A full report which will include national research and local data/information will be published on Nottinghamshire Insight once complete.
