Overview

In Nottinghamshire we are collecting feedback about health visiting and school health teams (known as your Healthy Family Team). This will help us make sure services continue to meet the needs of local children and their families. We would like to hear your views as a parent, partner, step-parent, grandparent or carer who uses the service. 

Some teenage and young parents have the support of a Family Nurse from the Family Nurse Partnership (FNP). This survey is about the Healthy Family Teams, not the FNP.   

It should take around 15 minutes to complete the survey. You do not have to answer any questions that you do not want to. We will keep all information confidential. The survey does not contain any information that can personally identify you.
