Overview

Nottinghamshire County Council are currently reviewing the NHS Health Checks programme, delivered by GP Practices for people aged 40-74 years old to assess their risk of developing heart disease, stroke, kidney disease or diabetes.
 
We would like to hear from you about your views and experience of the programme, whether it’s from attending or being a resident who has been invited to have one. This will form our planning to improve the programme over the next few months.

The survey will be open until 13 September 2024.
